Hi, I'm Jane Cranston, founder of Tiny Transitions Therapy Consulting PLLC. As a Board-Certified Pediatric Occupational Therapist, Certified Neonatal Therapist, educator, and mother, I understand that every child develops in their own unique way and that parenting can sometimes feel overwhelming, especially when developmental, feeding, sensory, or medical concerns arise.
At Tiny Transitions, I believe therapy is about much more than meeting milestones. It is about building meaningful relationships, empowering caregivers, and helping children thrive within the context of their everyday lives. My approach is family-centered, trauma-informed, and rooted in the belief that parents are their child's most important teachers and advocates. Rather than focusing only on what a child cannot do, I work alongside families to identify strengths, celebrate progress, and create practical strategies that fit naturally into daily routines.
With more than 25 years of experience supporting premature infants, NICU graduates, medically complex children, and families, my goal is to bridge the gap between clinical expertise and real-life parenting. Whether we're working on feeding, sensory regulation, development, or caregiver confidence, I strive to provide compassionate guidance, evidence-based care, and personalized support that helps both children and families feel successful. Virtual consultations and caregiver coaching services may also be available for families outside our primary service area.

Yes. We provide in-home pediatric Occupational Therapy services, allowing us to support your child within their natural environment and daily routines. Home visits also provide valuable opportunities for caregiver coaching and individualized support tailored to your family's needs.
Do You Offer Membership Plans and Affordable Pricing?
Yes. Tiny Transitions Therapy Consulting is a private-pay practice that offers flexible service options, including membership plans designed to make ongoing support more accessible and affordable for families.
Our membership plans provide personalized developmental guidance, caregiver coaching, and direct access to an experienced pediatric Occupational Therapist, allowing families to receive consistent support without the limitations often associated with traditional healthcare models.
We believe every family deserves access to expert care and are committed to helping families find a service option that meets their needs and budget. We also provide superbills for eligible services that may be submitted to insurance companies for possible out-of-network reimbursement.
