NEWBORN CARE & IMMEDIATE POSTPARTUM [x] Skin-to-Skin: [x] I request immediate, uninterrupted Skin-to-Skin contact for at least 1 hour after birth. [x] Feeding Preference: Combination Feeding (Bottle Feeding Only) [x] No Direct Latch/Breastfeeding: I prefer not to direct breastfeed / latch the baby. [x] First & Ongoing Feedings: Please provide a hospital-grade breast pump so I can express colostrum/breast milk to bottle-feed, supplemented with ready-to-feed formula via bottle as needed. [x] Bathing: Please delay baby’s first bath for at least 24 hours (or until discharged). [x] Standard Care: I consent to standard routine newborn procedures (Vitamin K shot, Erythromycin eye ointment, Hep B vaccine).
