LABOR & PAIN MANAGEMENT ​[x] Epidural: I plan to request an Epidural [ Early in labor / When I feel I need it ]. ​[x] Movement: I would like to use a birth ball / peanut ball and change positions frequently. ​[x] Medical Interventions: Please discuss with me before performing artificial rupture of membranes (AROM) or administering Pitocin. ​2. DELIVERY & PUSHING ​[x] Perineal Care: I prefer perineal support/warm compresses to minimize tearing. Please avoid episiotomy unless medically necessary. ​[x] Umbilical Cord: ​[x] Please practice Delayed Cord Clamping (at least 60 seconds / until pulsating stops). ​3. NEWBORN CARE & IMMEDIATE POSTPARTUM ​[x] Skin-to-Skin: ​[x] I request immediate, uninterrupted Skin-to-Skin contact for at least 1 hour after birth. ​[x] Feeding Preference: Combination Feeding (Bottle Feeding Only) ​[x] No Direct Latch/Breastfeeding: I prefer not to direct breastfeed / latch the baby. ​[x] First & Ongoing Feedings: Please provide a hospital-grade breast pump so I can express colostrum/breast milk to bottle-feed, supplemented with ready-to-feed formula via bottle as needed. ​[x] Bathing: Please delay baby’s first bath for at least 24 hours (or until discharged). ​[x] Standard Care: I consent to standard routine newborn procedures (Vitamin K shot, Erythromycin eye ointment, Hep B vaccine).
